Evacuation Plan Forms

On June 23, 2006, senate bill no. 607 became Act 615.  Act 615 was developed to formulate emergency operation plans for humane evacuation, transport, and temporary sheltering of service animals and household pets in times of emergency or disaster.

The act requires that animal shelters, humane societies, veterinary offices, boarding kennels, breeders, grooming facilities, hospitals, schools, animal testing facilities and any other businesses or not-for-profit agencies that normally house household pets or service animals to create an evacuation plan. This evacuation plan is to be filed annually with the Louisiana Department of Agriculture and Forestry (LDAF) and your parish office of homeland security and emergency preparedness.

Currently, the LDAF is requesting that this plan be filed with their office by July 31, 2006Plans must also be submitted to your parish office of homeland security and emergency preparedness. Please download the form from the link below.  If you are unable to and would like the LVMA office to mail you a hard copy of the forms, please contact the office.
